Directions for Completing SAT I and II Registration Forms

Most of the items on the registration form are self-explanatory, but there are some items we wish to call to your attention. Once you have registered by mail for any SAT test (other than the PSAT), you will be able to register online for future tests. Directions are essentially the same.
  • Make sure you meet the registration deadlines!
 
  • Use blue or black ink. Do not use a pencil. Print clearly in capital letters.
  • Items 1-4 ask for personal information.
 
  • In item #5, the high school code is 333900.
  • Item #8 can be answered as you see fit. If you are eager to receive a lot of mail from colleges (and we mean a lot of mail), then answer in the affirmative.
 
  • In item #9, indicate which test you want to take and the month that you plan to take the test.
 
  • In item #10, if you wish to take the test at Ramaz, fill in 33674 in the space for First Choice. Please note that our directions differ from the instructions given in the registration booklet. If you wish to take the test at a center closer to your home than Ramaz, fill in 01000 in the space for First Choice. If you are assigned to a test center you do not want, call the College Board at 866-756-7346 to ask for a switch. You should not be charged a fee if you are switching to Ramaz.
 
  • Unless you are a senior, you should leave items 11a and 11b blank. In item 11a you may select four colleges to receive all your SAT scores at no extra charge. In item 11b you may select additional colleges to receive your scores for $10 each.
 
  • Items 12 and 15 require personal information. Skip items 13 and 14.
  • If you would like to pay for the exam(s) by credit card, enter the information in item 16.
 
  • Fill in items 17-21 carefully. The College Board books that are being offered are all available for loan in the College Office.
  • On the back of the form, you need to complete and sign item #23. Item #22, the rest of the back, can be left completely blank. Information that you enter here is used by the Student Search Service. Only answer questions that you feel will portray you in a positive light.
  • Remember to include your check or the credit card information requested in item #16, as well as the clergy letter.
  • On the front of the envelope, fill in the oval for the month you plan to take the test and put the stamped envelope in the mail. You should receive an admission ticket in the mail about three to six weeks after your completed application reaches Educational Testing Service.
