Directions for Completing ACT Registration Forms
Most of the items on the registration form (called a "folder") are self-explanatory, but there are some items we wish to call to your attention. You may register by mail or online for any ACT test.
- Items A - K ask for personal information.
- The high school code in item L is 333-900.
- For item M1, you should choose “ACT Plus Writing.”
- In item M2, indicate the month that you plan to take the test.
- In item N, you must fill in test center codes. Test center codes are six digits long; codes that end in "1" are Sunday locations. For a complete list of the New York test centers and to find the one most convenient for you, please see http://www.actstudent.org/regist/centers.html or the list of test centers in the ACT registration booklet. Sunday Centers are listed at the end of each state's list.
- For the basic registration fee, you may have your scores sent to four colleges of your choice. Consult the list of colleges and their corresponding codes in the instruction booklet and indicate the four you choose in item O. You may wish to consult your college adviser before listing any colleges.
- Item P can be answered as you see fit. If you are eager to receive a lot of mail from colleges (and we mean a lot of mail), then answer in the affirmative.
- As required, please fill in the sections titled “High School Course/Grade Information,” “Interest Inventory,” and “Student Profile Section” on pages 2 and 3 of the registration folder based on the instructions on pages
13 – 18 of the instruction booklet.
- Items Q - V on page 4 of the registration folder require more personal information.
- Fill in item W if you are using a fee waiver or voucher. Please see page 5 in the instruction booklet regarding eligibility.
- Item X requires your signature and the date.
- Finally, item Y asks you to calculate the fee that you owe.
- Remember to include your check or money order (credit card information can only be used when registering online) as requested in item Y.
- Please mail the registration folder and the clergy letter in the 9” x 12” envelope provided.
- Put the sticker for the month you plan to take the test on the front of the registration envelope and then put the stamped envelope in the mail. You should receive an admission ticket in the mail about three to six weeks after your completed application reaches the ACT.
